The DiRECT trial — the landmark structured weight-management trial in Type 2 diabetes — just published its 5-year follow-up, showing that remission is possible and durable when weight loss is maintained. At 1 year, 46% of participants in the intervention group were in remission, per the DiRECT primary trial report in The Lancet. At 2 years, 36% remained in remission, per the DiRECT 2-year follow-up. At 5 years, 13% of participants in the extension cohort were still in remission (11 of 85), with average weight loss of 6.1 kg and remission strongly tied to sustained weight loss, per the DiRECT 5-year follow-up in The Lancet Diabetes & Endocrinology. For the 38 million Americans living with Type 2 diabetes, this is being framed as the moment remission is proven to be achievable over the long term. It is — for patients who sustain meaningful weight loss. It is not, and cannot be, a cure that happens automatically.
What Just Happened
The DiRECT 5-year extension extended the original trial — the largest randomized controlled trial of structured weight management for Type 2 diabetes remission — out to 5 years, as detailed in the PACE CME summary.

- At 1 year: 46% of intervention-group participants were in remission (A1c < 6.5% off all diabetes medication), versus 4% of control-group participants, with mean weight loss of 10 kg
- At 2 years: 36% of intervention-group participants remained in remission, with mean weight loss of 7.6 kg. Among those who maintained >10 kg weight loss at 2 years, 81% were in remission.
- At 5 years: 13% of participants in the extension cohort were still in remission (11 of 85), with average weight loss of 6.1 kg below baseline.
- Among those in remission at 2 years: 26% remained in remission at 5 years.
- Mechanism: Remission at every timepoint was strongly correlated with sustained weight loss, not baseline characteristics or intervention assignment alone. Participants who maintained >15 kg of weight loss had >80% remission rates; participants who maintained >10 kg of weight loss had 75% remission rates, per the DiRECT follow-ups.
- Best responders: Participants with shorter diabetes duration at baseline (<6 years from diagnosis) were more likely to achieve and sustain remission
- Implication: Remission is achievable — but requires sustained effort. The 5-year follow-up shows that the disease can come back if weight is regained, per the DiRECT 5-year follow-up.
The Question Nobody’s Asking
Here’s what every headline about the DiRECT 5-year follow-up is leaving out: remission at 5 years is real, but only for the 13% of extension participants who sustained meaningful weight loss.
The DiRECT trial is the strongest evidence in the world that Type 2 diabetes remission is achievable through
structured weight management. 46% remission at 1 year is extraordinary. But the 5-year follow-up tells the
harder story: most participants regained weight over time, and the overall remission rate fell from 46% to 13%
over 5 years in the extension cohort. The patients who sustained remission were the ones who kept the weight
off — and that’s a much smaller group than the headline suggests.
